Identification of Focus for Community Teaching (Topic Selection):

***Prevention of the progression of Chronic Kidney Disease in African American Population.

Epidemiological Rationale for Topic (Statistics Related to Topic):

**“CKD and ESRD are very costly to treat. Nearly 25% of the Medicare budget is used to treat people with CKD and ESRD. It is estimated that CKD affects approximately 15% of the general adult population in the United States.” (Flavis,2020, February 4). Chronic Kidney Disease is recognized as a growing epidemic all over the world that has a major impact on quality of life.

Teaching Plan Criteria

Your teaching plan will be graded based on its effectiveness and relevance to the population selected. This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion. 

Nursing Diagnosis:

**Fluid excess related to decrease glomerular filtration rate and sodium retention as evidence by edema, hypertension, weight gain and shortness of breath.

 

Readiness for Learning: Identify the factors that would indicate the readiness to learn for the target aggregate. Include emotional and experiential readiness to learn.

Determine what the individual need to know and find the time when they are ready to learn.

**Apply interpersonal communication is essential for successful teaching.

**Know their motivation

**Patient’s willingness to learn.

**Health status of the patient for effective learning.

**Time and place of the teaching session.

**Type of learning style.

**Behavior of the patient.

**Involvement of family’s members.

 

Learning Theory to Be Utilized: Explain how the theory will be applied.

 

**Social learning theory: Provides one of the most useful approaches to individuals education because it explains the characteristics of the learner and guides the educator in developing effective teaching intervention. For example: self-efficacy is a strong predictor of healthy behaviors and because nurses and other health care providers can implement interventions that improve self-efficacy, which results in lifestyle choices.

 

Goal: Healthy People 2020 (HP2020) objective(s) utilized as the goal for the teaching. Include the appropriate objective number and rationale for using the selected HP2020 objective (use at least one objective from one of the 24 focus areas). If an HP2020 objective does not support your teaching, explain how your teaching applies to one of the two overarching HP2020 goals.

**CKD-1. Reduce new cases of chronic kidney disease (CKD) and associated complications, disability, death, and economic costs.

**Rationale: to improve the quality of life for individual with CKD.

**Rationale: To decrease disparities among kidney disease patients and to decrease the medical cost used to treat individuals with chronic kidney disease.

 

 

 

 

 

How Does This HP2020 Objective Relate to Alma Ata’s Health for All Global Initiatives

**8Alma ata in the year 1978 came with declaration of health for all by 2000. The aim of this declaration was to urge all the national Governments to paid attention on providing health as a major basic to all. It highlighted on making policies and protocols that will assist in reestablish and preventive health care aspects as well as to increasing the use of resources for the health care. Also assisted in decreasing the gross inequality among the people of developed and under-developing countries

Strategies/Methods

Example – Interactive poster presentation of the Food  Pyramid. After an explanation of the poster and each food category, allow students to place pictures of foods on the correct spot on the pyramid. Also, have the class analyze what a child had for lunch by putting names of foods on the poster and discussing what food group still needs to be eaten throughout day.

**1.African American population, will name a list of healthy food that is low in fat content, total calories and processed foods, and high in fruit and vegetables to decrease the risk of develop diabetes

 

 

 

**1.Healthy foods guide that are low in fat, low calories are: Green veggies, like broccoli, spinach, and any other dark leafy greens.

**Eat more orange vegetables like carrots and sweet potatoes.

**Eat more dry beans and peas like pinto beans, kidneys beans and lentils.

**Eat a variety of fruit: fresh, frozen, canned or dry fruit.

**Drink low fat or fat-free milk and yogurt.

**Choose low-fat or lean meats and poultry.

 

1.After the distribution and explanation of the informational brochure the patients and their families will:

**- Give a list of healthy food choices.

**-Provide a list of food planner for the next three days after first day of reading and completing the education session from the presenter.

**2.African American population will list lifestyle changes that aids to decrease high blood pressure.

 

 

 

**1.     2. Lifestyle changes that aids to decrease blood pressure are: Lose extra pounds and watch your weight.

**2.     -Take B/P medication daily (as prescribed)

**3.     Exercise regularly

**4.     Eat a healthy diet

**5.     Reduce sodium in your diet & caffeine.

**6.     Limit the amount of alcohol you drink.

**7.     Quit smoking & reduce stress.

     

 

**2. Individuals will monitor blood pressure daily.

**-Measure B/P at same time.

**-Take multiple readings and record the results.

**-Report any blood pressure greater than 140/90 to health care provider.

**-Will have a list of healthy food choices.

**-Will report no alcohol and caffeine consumption.

**-Will report no smoking and using breathing techniques to reduce stress.

**3.African American population will give a list of community resources and weight loss programs to achieve normal weight as well as to list all exercise activities that aid weight control

 

 

 

*83.Goals for obesity control are:

**Consult a registered dietitian to ensure a success.

-**Engage in exercise activity daily or minimum 2-3 per week.

**-Start simple by using my plate app.

**-Use food diary to monitor daily intake.

**-Monitor BMI

**-Monitor daily nutritional values, including fat intake.

**-Removes sugar beverages from diet.

**-Monitor blood sugar levels.

**3. -Individuals will do more than two hours and 30 minutes (150 minutes) of moderate-intensity aerobic activity (brisk walking) every week or 1 hour and 15 minutes (75 minutes) of vigorous intensity aerobic activity (e.g., jogging or running) every week or an equivalent mix of moderate- and vigorous-intensity aerobic activity every week. – Muscle-strengthening activities that work all major muscle groups (legs, hips, back, abdomen, chest, shoulders, and arms) on 2 or more days a week.

**When needed use the stairwells instead of the elevators.

**-Individuals will monitor waist circumference.

**-Individuals will have completed a list of food diary.

**-Individuals blood sugar level will be under 130

*4African American Population will give a list of family’s members with co-existing CKD to evaluate the risk of genetic disorders.

 

**4 Early screening tests for CKD are:

**-eGFR (estimated glomerular filtration rate)

**-Urine test (to test for protein in the urine)

**-Blood pressure measurements.

 

**4 Individuals will describe:

– A normal eGFR of 60 or more.

A normal UACR is less than 30mg/g.

Will report B/P readings of 120/80

 

 

Creativity: How was creativity applied in the teaching methods/strategies? 

 

The interaction among each other and the freedom of expression that individuals will enhance during this presentation. Given the opportunity to ask questions.

 

Planned Evaluation of Objectives (Outcome Evaluation): Describe what you will measure for each objective and how.

**1.     Individuals will provide a list of healthy foods choices and describe the quantity of food consume daily.

 

**2.     Individuals will report having B/P parameters no greater than 140/90 for three consecutives days and will report to maintain normal B/P parameters for the next three months.

 

**3.     Individuals body weight will decrease to 5% in 30 days.

 

 

**4.     Individuals eGFR will be >60 within the next 30 days.

 

Planned Evaluation of Goal:  Describe how and when you could evaluate the overall effectiveness of your teaching plan.

**1-    Asking individuals to demonstrate and provide feedback on the planning goals by the given deadline.

**2-    How well is the individual able to answer questions about the topic.

**3-    Did the individual value the information provided?

**4      Having individuals verbalize the risks factors for Chronic Kidney Disease.

**5-    What barriers prevented learning or change in behaviors.

**4-    Ensure to document the outcomes of the teaching plan.

 

Planned Evaluation of Lesson and Teacher (Process Evaluation):

Will evaluate the appropriateness of the intervention selected and the correct application of intervention. Base on the standard of care for an individual health problem.

 

 

Barriers: What are potential barriers that may arise during teaching and how will those be handled?

 

**Physical and mental condition. Physical discomfort is a major barrier to learning, both by the power of distraction and by biological factors such as pain

**Environment, lack of confidence, previous bad experience, fear of failure, lack of motivation and fear of change.

**I will ensure that individuals will be comfort and free of pain and ask to stop me at anytime if needed. I will continue monitoring their readiness to learn, by paying attention for their non-verbal messages.

 

Therapeutic Communication

4.2 Communicate therapeutically with patients.

**Active listening, seeking clarification, giving recognition and using silence.

 

How will you begin your presentation and capture the interest of your audience? Describe the type of activity will you use with your audience to exhibit active listening? Describe how you applied active listening in tailoring your presentation to your audience? How will you conclude your presentation? What nonverbal communication techniques will you employ?

 

**1-    Will introduce myself and ask them what they want to know about Chronic Kidney Disease.

 

**2-    Explain the audience how this presentation will benefit them and why the information is valuable for them.

 

**3-    May ask if someone knows the tips for Chronic Kidney Disease prevention and use a body language to convey a sense of self-assuredness

 

**4-    I will ask questions in between the presentation to assess for active listening.

 

**5-    Give some time to ask to follow up questions and thank you them for their time.
