Ms ABC Hispanic female referred to ED by ophthalmologist for an acute onset of thrombocytopenia. Current meds include: Prednisone 20mg BID, Methotrexate 20mg weekly, Zantac 150 mg BID, Vigamox 0.5% ophthalmic 1 gtt in the Right eye BID, erythromycin 0.5% ophthalmic ointment Right eye QHS, Ibuprofen PRN, folic acid 1 mg daily, metformin 750 mg daily. she was started on methotrexate approximately 6 weeks ago, but has been off the med for 2 weeks, and just got it refilled yesterday at her appointment with her ophthalmologist which is when labs were completed. She took a dose last night (after labs were done). She has not had any overt signs of bleeding.

Assessment: Vitals stable, examination negative ( skin, heart, lungs, abdomen, rectal, stool for occult blood negative). Has some scattered petechiae.

Labs: NA+ 125, K+ 4.2, CO2 20, Ca 9.4, Cr 0.9, BUN 20, Glucose 573, Albumin 3.6, AST 16, ALT 33, WBC 8, Hct 39%, Hgb 13, MCV 92.8, platelet 7000.
