After posting a short narrative introduction to your proposed quality improvement idea, you will list an initial draft of each of your five PICOT elements:  

·    P: Identify/define the population   

oThis can be the population (learners) that is being asked to change a practice. 

oThis may be a population (learners) that is the focus of the change.  

oDo not use this element to state the problem.  

·    I: Intervention – This is the innovative educational intervention/teaching strategy you propose to address the problem and is intended to produce an improved outcome.  

·    C: Comparison – What is the current practice that is not producing the desired outcome? This could be “…” compared to no intervention…” 

·    O: Outcome – This is an objective measure that will be used to evaluate the effect of your intervention. There must be objective and measurable metrics as part of this statement.  

·    T: Time – How long will it take to have enough time and enough data to determine if your intervention produced the desired results? You should not exceed 8 weeks, as you will be implementing this quality improvement project in either NR621 or NR622. 

For example, let’ say that you are in a pharmacology didactic course for your practicum site and your mentor states that students are exhibiting low test scores. The class is currently being taught primarily by lecture. You and your mentor decide to implement a flipped classroom to see if the students understand the concepts better. Your PICOT question could look like this: 

In first year nursing students enrolled in a pharmacology course (P), will a flipped classroom (I) as compared to no intervention (C) improve students’ knowledge related to the identified concepts (O) in eight (8) weeks (T)?

Module 5 Key Terminology: Prosthesis and Implants

Cybernetics: the science of how a system communicates information to create automatic processes.  

In a cybernetic system, information is collected, a decision is made based on the information, action is taken based on the decision, and changes are made in the world. Information is collected based on those changes, and the loop continues. 

A thermostatis an example of a cybernetic loop: once you set your thermostat to, say, 72 degrees, it collects temperature data from your house and communicates to the heating/cooling system how much hot or cold air to blow out to maintain the temperature at 72 degrees. Human and animal bodies do similar things: we collect sensory data through our skin, eyes, ears, nose, and mouth, and our brain automatically responds to allow us to move around in the world.

Prosthesis: an artificial body part or an implant that allows biological body parts to function normally

Cyborg:A person whose body has been augmented or enhanced with technology. Technically, a person with any form of prosthetic body part is a cyborg.

B)  The first-ever World Patient Safety Day is taking place on Sept 17, 2019. Every day, countless patients worldwide are put at risk by unsafe care and end up requiring treatment for ailments caused by the very system that was supposed to help them get better. Protecting patients from errors, injuries, accidents, and infections is an essential goal for every health system, but no health system has so far successfully addressed patient safety.

Some of the statistics proffered by WHO to highlight patient safety are striking. In low-income and middle-income countries (LMICs), 134 million adverse events per year are directly attributable to unsafe care. These adverse events—including misdiagnosis, hospital-acquired infections, and medical errors—lead to 2·6 million unnecessary deaths. Worldwide, the risk of patient death because of a preventable medical accident is one in 300. One in ten patients suffer injury while receiving health care, and 15% of all hospital expenses are incurred as a result of treating failures in patient safety.

Patient safety hinges on quality of care. The Lancet Global Health’s 2018 Commission highlighted the need for “high-quality health systems that optimise health care in each context by consistently delivering care that improves or maintains health”. It feels obvious to state that a health-care system should aim to improve the health of those accessing it. Similarly, all health professionals expect that patients will have their condition improved by health care. However, the data compiled by WHO should be a wake-up call as they would be in any other industry. So what can be done?

First, do no harm. The safety of patients must be the paramount concern of professionals and the systems they work in. Rather than a platitude, this ask is an exhortation to strengthen systems, build better infrastructure, and value strong leadership. Reporting in US hospitals shows some health-care-associated infections can be reduced by as much as 70% with proper patient safety interventions that include standardised clinician education, proper notification processes, and strict hand hygiene procedures. However, the WHO hand hygiene guidelines suggest compliance with proper hygiene can be as low as 40%. Hence, a greater effort needs to be made in monitoring and ensuring that basic practices of patient safety are strong and robust across all institutions, no matter how obvious the need for such procedures.

Second, health professionals must recognise that patient safety is a two-way partnership. Patients must be involved—indeed be central—in their own care. The myriad ways inadvertent harm can be done to patients indicate that everyone, from policy maker and health advocate to caregiver and health worker, holds a vital stake in patient safety. Indeed, evidence suggests that involving patients, service users, and carers in important decisions relating to care and treatment strengthens patient safety and is the best way for patients to achieve a positive outcome. As WHO comments, “safe health care starts with good communication”.

Finally, awareness of the burden that patient safety requirements place on LMICs is needed: addressing all improvements necessary for increased patient safety require resources. Two-thirds of all adverse events resulting from unsafe care occur in LMICs. Health professionals in high-income countries must ask themselves what they can do, not just to promote patient safety in their own system but also to offer outreach, support, resources, and expertise to LMICs bearing the burden of raised patient safety standards, rapidly changing disease patterns, and expectations of achieving the same development goals.

World Patient Safety Day is a prompt to everyone involved in care to examine their role in contributing to these drivers. In the treatment of immediate illness, health-care systems must offer best practice and consistent treatment for all patients, and at all levels, to ensure further damage is prevented.

Artificial Intelligence: Any machine or software that effectively mimics human intelligence, including thought patterns, speech, a decision-making. Essentially, a robot is hardware (body/machinery), while AI is software (mind/program).

Robots and androids may or may not be artificially intelligent, so it’s important not to conflate the terms. In science fiction, AI is often depicted as sentient or having consciousness like humans. AI in reality is not sentient but does a good job of mimicking human speech or movement.

 

Alexa is not sentient because she has no consciousness or sense of self like humans do, but the software used for Amazon Alexa is artificial intelligent speech processing.  An Echo is literally a microphone you keep in your house, which Amazon listens to. When you give Alexa a command, the software processes what you said and responds in kind, but it also collects information on your requests. The software stores your past requests to try to guess what else you might want and get you to buy new things. This is why you sometimes talk about a new product and then receive ads for that product later that day. To stop this from happening, make sure you set your phone to disallow microphone use by applications. It’s also best just not to have an Echo or other “smart” home device. 

 

 

Cyborg: A person whose body has been augmented or enhanced with technology.

Foe example, Neil Harbisson has a device implanted in his head that allows him to hear color. We’ll talk more about him in a later module.

How will you use critical thinking in your evidence based practice, as a nurse, and as an informed citizen?

Background: Critical thinking is a habit of mind. That means that it’s not a set of facts you can learn or a routine procedure you can memorize. It’s a way to train your mind whenever it encounters new information. When you learn to think critically you are changing the way that you think. One of the goals of this program is to give you practice thinking critically about a variety of different topics and in a variety of different contexts. If you become a critical thinker on the job as a nurse, you cannot help but become a critical thinker in your personal life, and vice versa. Remember, habits get stronger and more easily used the more you practice them.

So, this week you’ll read a chapter from a book called The 5 Elements of Effective Thinking about “Being your own Socrates.” If you’re unfamiliar with who Socrates was, just remember that he was a philosopher who made an art out of asking really effective, challenging questions. No one he talked to could get away with making a claim that was not completely defined and supported by logical evidence. This kind of questioning is the foundation of evidence-based practice. You’re being asked to watch a movie because it this will hopefully illustrate that critical thinking can be utilized in every aspect of your life – professional and personal. 🙂

Instructions:

1) Read “Be your own Socrates” and take notes on it

2) Think of times in your personal or professional life that you have acted as your own Socrates

3) Watch your favorite movie. Even if you’ve seen it 30 times, watch it again. And as you watch it, take notes using Burger and Starbird’s guidelines for being your own Socrates by asking questions. As they suggest, “actively construct well-founded questions about what is missing, what is assumed, what might be extended, or what is vague or unclear.”

4) In your main post, begin with an introduction, then  LIST the questions that you came up with for your favorite movie, and then answer the following questions:

  1. a) How did it feel to ask probing questions about something that you thought you already knew a lot about? What did you learn about this film and about yourself as a viewer from asking critical questions about this film? What topics or issues did you realize you could learn more about by asking questions of this film?
  2. b) Describe a situation in your professional or personal life in which this habit of asking probing questions would benefit you? In your answer, keep in mind that EBP requires you to find reliable evidence to support any claim or decision you make in healthcare, but that in order to find the right evidence, you need to first ask the right questions.

Hess (2013) did a study of great leaders. He looked to such high performing organizations as Chick-fil-A, Home Depot, and the US Marine Corps. Leaders who performed well were not necessarily charismatic and commanding. Rather, he found that leaders who did not forget what it was like to be on the line were effective. Interesting question this week! Credibility is defined as: “the quality of being believable or worthy of trust” (“Credibility”, n.d., para 1). We have addressed trust in many other weeks. We are now adding the concept of being believable. I think the concept of being believable to being authentic. It is interesting; most people can see through non-authenticity. We have seen this when actions do not match words. We see this when body language is not consistent with eye mannerisms. There are many leadership styles and theories available for use. Dr. Hersey developed the Situational Leadership model in the 1960’s. This model provides a framework for a leader to use with followers dependent upon:

  • The amount of guidance and direction (task behavior) a leader gives
  • The amount of socioemotional support (relationship behavior) a leader provides
  • The readiness level that followers exhibit in performing a specific task, function, or objective (Center for Leadership Studies, n.d.).

An example would include development of a new nurse as opposed to a nurse with vast experience. The leader would need a more hands on, authoritative approach in times of crisis versus an experienced nurse who could be better supported by a transformational leader. Leaders in a code need to be authoritative as opposed to a patient who needs wound care.

 

All leaders need to be flexible in their approach with employees; styles should be based upon employee needs.

This is a very interesting question since many aspects of the two paradigms are similar. The base root of transformational to “transform”, meaning to change. Unlike traditional leadership models, transformational leadership (very similar to Servant Leadership) is all about employee growth. Transformational leaders focus on employee development. They provide an environment where employees can learn new skills and “transform”, even to the point possibly where they will move out of the organization. Transformational leaders have been linked to high-performing team, staff retention, and improved patient outcomes (Fischer, 2016). In fact, transformational leadership is one of the pillars of AACN Magnet status.

Such a great question for us this week! I work in hospice. We frequently use the words “love” and “care” as verbs instead of feelings. When used as feelings, people immediately think about “soft” science. However, if we move into the realm of action (as a verb), these concepts become powerful. Van Dierendonick and Patterson (2015) propose that a leader’s “propensity for compassionate love will encourage a virtuous attitude in terms of humility, gratitude, forgiveness and altruism” (para 1). How does this statement translate to leadership action? What Are the Key Differences between Naltrexone and Naloxone (Narcan)?

Because the two drugs have similar names, naltrexone and naloxone are commonly confused. Both of these medications are opiate antagonist drugs, meaning that they prevent or block opiate drugs from reaching the brain at a cellular level. Opiate drugs include numerous prescription medications that are primarily used to control moderate to severe pain as well as several illicit drugs. All opiate drugs are either processed from the opium in the Asian poppy plant or synthetically produced chemical substances that are similar to substances found in opium. This is a large class of drugs and consists of prescription medications like morphine, Vicodin (acetaminophen and hydrocodone), OxyContin (oxycodone), codeine, etc., as well as illicit drugs like heroin.
