Instructions:

Answer the questions below to create an informed consent document for your clients. Consider where you are in your career and the types of clients you will be working with. On page 85 of your textbook, Moyer & Crews describe informed consent as “a counselor’s way of providing the who, what, when and how of counseling.” Text Box 5.1 (pp. 85-86) and 5.2 (p. 87) include important details about informed consent. Use chapter 5 in your textbook to help you answer the following questions:

  • What are the goals of the therapeutic process?
  • What services will the counselor provide?
  • What is expected of the client?
  • What are the risks and benefits of therapy?
  • What are the qualifications of the provider of the services?
  • How long is the therapy expected to last?
  • What are the limits of confidentiality?
  • When might the therapist need to refer a client because of personal values?
  • In what situations does the counselor have mandatory reporting requirements?

When answering the following questions, please use headings for each section. For example, question 4 asks about risks and benefits. You might use Risk and Benefits as your heading for that section of the informed consent. Please do not use the questions as headings. You are working towards an actual informed consent that you can use in your counseling practice.

Your informed consent should be 1-2 pages in length.

Each section is worth 10 points, which equals 90 points. You will earn the last 10 points by adding a simple, yet necessary component to the end of your informed consent. APA format is NOT required for this paper unless you are citing someone’s work. Therefore, no title page or reference page is necessary.
