Great post Richard, i can also add that , Carteret (2011) in her article on Cultural values of Asian patients and families, she mentions that Asians tend to be highly group-oriented people who place a strong emphasis on family connection as the major source of identity and protection against the hardships of life.  I have interacted with Asians American immigrants from India and Philippines before and I totally agree with this statement.  They are family oriented kind of people and their main focus is the family.   When interviewing a 20 year old Asian American, Carteret (2011) emphasizes that one need to establish the professional role and assume authority, check for understanding, be patient and consider periods of silence opportunities for reflection, provide clear and full information, be attentive to nonverbal cues, address immediate needs and give concrete advice and reach consensus by compromising.  One more point I like from her article,  (Carteret, 2011), is that an intentional lack of directness in conversation is favored by people from many Asian cultures because preserving harmony between people is often more important than getting at the exact “truth”.  “Checking for understanding when communicating with Asian patients/families is critical,” Ms. Carteret says. “It is all too easy to misinterpret a common gesture as agreement or understanding when the patient is actually confused or even resistant to a diagnosis/treatment.

Instead of every 10 years for adult booster immunization, the CDC now says every adult patient admitted to a hospital for surgery should have had a Td booster in the past 4 years. (NOTE: This policy change is fictitious, used only for the purposes of this exercise.)

Your quality manager knows that you, as health information manager, have data on hospital admissions and might have information in the records on immunizations. She wants an informative report, not just the raw data. You will develop this report on a historic sample of patients to predict possible effects of this policy.

At first, you scratch your head. It could take a long time to count back days from the hospital admissions. You figure out you need to compare the hospital admission date with the tetanus immunization date. You check your data. Fortunately these data are saved as date fields. You should be able to compute the information.

Your task:

Adult patients are those over age 18. For the purposes of this analysis, assume that all patients in the database were surgical patients.

  1. Download the Patient Information Excel datasheet for the sample of patients.
  2. Open the spreadsheet.
  3. Add one column on the right called Days Td to Admit. Add another column to its right called, Years Td to Admit.
  4. Compute the Days Td to admit by subtracting Tetanus Date from Admission Date. Compute the Years Td to admit.
  5. Determine which patients and how many would not have complied with the new policy because their boosters were not recent enough. What is the rate of patients out of compliance, as a percentage of the total sample? Remember to exclude children.
  6. Create a new worksheet in the Patient Information Excel datasheet and prepare a table of patients who would be out of compliance, including patient names, birthdates, admission dates, and number of years Td to Admit for the quality manager. Indicate this information was from a sample, not the entire hospital.

According to Healthypeople.gov, (2017), social determinants of health are situations which involve social connection such as religious places, mosques, temples, work places, learning institutions, places of play, people are born, and age. All these social determinants contribute to the development of illnesses. Health begins with our immediate contact social/physical environment; churches, schools, communities, hospitals, and at home. The quality, safety, and cleanliness of these places contribute to our being healthy alongside staying active, and eating healthy. Patients who have respiratory illnesses may say they worked in environment where there was air pollution/contamination which they were exposed for a long time, if quality of food that people are ingesting is not healthy, then development of heart diseases, or diabetes is inevitable. A community that is exposed to crime, violence, and insecurity creates social disorder, if the quality of education, and training is poor, then the community members are not able to secure well-paying jobs, resulting in poor housing, for example people that live in shelters due to not being able to secure a well- paying job, own, or rent a house, would be at risk to being exposed to various illnesses that other people have.

The communicable disease chain is described by Center of Disease Prevention, and Control, (2017), as a chain of infection which includes; host called reservoir (infected agent), port of exit (through which the pathogen leaves the infected host), mode of transmission (direct or indirect), and port of entry (is how pathogen enters the susceptible host).
