10 Warning Signs You Are Working with the Wrong Nurse Manager

Manager Finding the right nursing job includes finding a manager that will help you grow, develop, and support your career goals.

There is a saying that people leave their managers and not their jobs and to have success in your career may mean you want to circulate your resume in order to find the best fit for you. This means you have to know what you want and need in the area of support.

Here are 10 warning signs you may be working with the wrong manager:

  1. You never hear from your manager prior to your performance reviews. Over 75% of performance problems can be improved with proper feedback and less than 33% of the time, feedback is provided.
  2. You have no idea what they want.  This can be worse than not having feedback at all. When a manager says, “I do not like how you did that,” you really have no way of knowing what they really mean. Make sure to ask for clarification. Review your job description and ask for your manager to specify what parts of your job responsibilities are most important to them. It could be they are focused on patient safety and you have an interest in health literacy. Knowing what they want gives you the advantage of focusing your efforts for the greatest gain.
  3. It is their way or the highway.  This is a problem for many nurses. Job satisfaction comes with autonomy and the opportunity to solve your own problems as they show up on the job. When a manager consistently tells you what and how to do something, employees quickly turn off their own creativity; more than likely, with an increase in mistakes.

    A nurse who is practicing for 7 years shared a story about his experience on a new unit. His Clinical Specialist was a micro-manager. She told him to give this medication immediately because of incoming admissions. She had poured the med. This went against his better judgment but because he knew she would have a fit, he gave it, to the wrong patient. She was extremely apologetic however the “error” was on him. Do not compromise your judgment for the sake of status quo.

  4. Your manager wants you to figure it out.  The opposite of micro-managing is to not manage at all and letting everyone figure it out for themselves. This happens quite a bit leaving the power position to go to the most domineering individuals on the unit. Everyone needs to know the manager is in charge and when needed will make those tough decisions.
  5. You could not recognize them if your life depended on it.  If your manager hides behind email or a closed door, having a relationship with your manager will be impossible. Communication and trust is the foundation of a great working relationship.

    Suggestion for managers: Time is an important commodity and getting around to all your staff can be time consuming. Why not use technology and set up a short video. Most iPhones take excellent video. Take 2-3 minutes every week and share what is going on. You may also want to share something personal about yourself; if you just started juicing, kickboxing, celebrated an anniversary or a milestone with your children. Interview them, show images along your morning run, and share something of yourself in order to make the connection with your staff.

    Suggestion for staff: Make a short video on your unit of a new initiative, gratitude board in the break room, more efficient way to give report, a snippet of rounds, and just a friendly hello from everyone on the unit. You may even want to say thanks and express appreciation; managers are people too!

  6. The way out the door is faster than up. If you have a manager that makes any attempt for you to advance your skills difficult, it will be hard to boost your resume. Managers can feel threatened by qualified staff members who want to move up the ladder and may indirectly thwart your efforts to move forward. This is really short sighted on the manager’s part because any manager that turns our qualified leaders actually looks really good to their higher ups given the ongoing need for good talent in any organization.
  7. Lack of training.  Being able to do a job well requires the right training. Often it takes the manager to assess the need for training based on performance and outcomes. This relates to the lack of feedback. While every employee really should do their own assessment of what they need to do well and then make the request of their manager, the manager should also be on the lookout for staff that need training and set up opportunities to make this happen.
  8. When the manager has obvious favorites.  Everyone has preferences in personality style, but when the manager consistently selects one particular individual for all the initiatives, opportunities for advancement, or other assignments that provide variety, the manager is sending a message to others they do not care about your skills or your future.
  9. When your manager routinely says, “I’ll think about it.”  Obviously considering all sides of the problem/ situation is important however some managers hide behind this and never make a decision about what is the ideal way to go.  Quickly, this can be frustrating if you are looking for a course of action to solve a problem.
  10. When your manager over reacts or criticizes you in front of others.  This is a toxic behavior and is an indication you want to find a new place to work. Quickly this will diminish your self-esteem, leading to resentment and stagnation.

    Knowing what you want in the way of workplace is key and will help you avoid a poor manager. What type of opportunities are you looking for in the workplace? What are your career goals? Evaluate the workplace, ask questions, find out the management style, review a performance appraisal, ask about turnover, and see if you can build a relationship with your new manager.

 

Get the Job Before Your Interview Starts!

Interview Changing jobs can be a stressful process for some because of the dreaded interview process. But there are a few pretty simple tips that can help put you, and your interviewer, at ease to make for a much better experience, and lead you closer to that new job you’ve been wanting! Stop letting the interview process paralyze your career.

For most people, the worst part of thinking about changing positions, or getting a new job, is the interview process. Maybe you are someone who gets nervous and sweats, shakes, or just can’t focus on the questions being asked, which makes the interviewing process torturous for you, something you dread, and guess what?

If you feel awkward and uncomfortable, so does the person interviewing you. As someone who has moved around quite a bit, interviewed for numerous jobs, and scored an offer each time (not to toot my own horn), I have learned some tricks to interview well, that are applicable to most people.

And as someone who now interviews applicants, I have a whole new perspective of what and interviewer “sees” during an interview; there are some small Do’s and Don’ts that can make you appear more poised and ready than you may feel!

DO Dress the Part:

Even though you may be coming in for an interview for your first job as a nurse, Medical Assistant (MA), Patient Care Technician (PCT), etc. you want to dress as if you are coming in for a job as a Director or the Chief Nursing Officer. I am not telling you to spend a ton of money on some fancy suit, but you want to look nicely put together with clothes that fit you well and look nice.

I was walking from my car to an interview and I was wearing these great fitted pants that I found on sale at one of my favorite stores and couldn’t believe they were 60% off, they looked great, fit great, and with a top that I already owned, and a pair of smart black shoes, I felt (and looked) like a million bucks.

Until I tripped a little, looked down, and realized the hem gave away on one of my pant legs (probably why such an amazing pair of pants were on such a super sale in my size), so I acted quickly, hobbled quickly to my car, did a little “runway” hem with some tape that I had in my car (i.e. I taped up the hem inside of my pants), and went back on my way.

Even though it was a bit of smoke and mirrors show, no one knew that my pants were taped together, and I even got complimented on how great my outfit looked. You don’t need to spend a lot, to look like a lot, but looking neat in nicely fitting clothes, shows that you are putting in the effort to put your best foot forward and show yourself in the best light.

DON’T Dress for a Night Out or a Day of Work:

If you are applying for a clinical job, yes it is awesome that we get to wear scrubs to work everyday, and it does make those of us that work clinically, at a deficit for “business” attire in our wardrobes, but it doesn’t make it acceptable for us to wear scrubs to an interview. You also want to make sure that you aren’t wearing something that you would choose to wear out to a bar/nightclub with your friends.

I have seen quite a few outfits in my time that make me think twice about the applicants common sense. Don’t make the interviewer question your common sense; that means you have set yourself up to have to prove your intelligence and critical thinking skills, despite what your resume might say!

(I realize I put this in twice, but I can’t tell you how many times I have seen people really inappropriately dressed for interviews!!)

DO Have Good Posture:

Did you know that sitting straight up and keeping your shoulders back make you appear smarter, attentive, and more of a leader?

Well, it does. I may be interviewing you for a position in an entry level, but I am more apt to hire someone that shows me they can be a leader within their position and will work hard and role model their leadership skills. And if they stay in their position, they will hopefully move up the ranks quickly.

DON’T Oversell Yourself: 

A big mistake interviewees make is overselling their skills. If you don’t have a certain skill set for a job you are interviewing for, that’s OK. Not everyone is an expert in their field when they first start, right?

We all start somewhere. So when you are asked, for example, “How comfortable are you taking care of a patient on with an intraaortic balloon pump?” and you think “A WHAT?!?!?”

Don’t sweat it, and give an honest response such as, “I haven’t had the experience of taking care of such a patient, but I have extensive other skills, such as [insert skills here] that I learned quickly, and I would love the opportunity to learn more about those patients and their specific needs. Is this a common patient type on your unit?”

You do two things with that answer…

You let me look back at your resume to review your skills, and you also show that you are interested in this experience and willing to learn. I may be looking for a more experienced nurse, but I will definitely consider you and your willingness to learn as a huge asset; I would rather hire someone motivated to learn and improve than someone who is stagnant in their learning process and no longer feels excited about their role.

DO Be Honest on Your Resume: 

Sometimes it is glaringly obvious when people tell mistruths on their resumes, and sometimes it isn’t, but it usually becomes obvious during an interview. I have had perspectives that added some skills to their resume that they don’t have, and through standard interview questions, it got quite awkward as I realized they did not have the skills they boasted about. (see don’t oversell yourself!)

DO Be Positive

As with all experiences in life, if you walk in feeling positive, and confident, with a big smile on your face, and an open mind, you can win over almost anyone! There is no need to be nervous as the worst thing that can happen is that the job isn’t a match; so think positively and imagine that you already have the job, and your interview will be a great experience.

5 Things Labor Nurses Want You To Know

Labor As a labor and delivery nurse, here is what I wish I could say to every mother out there, what I’m sure many of us would want to say to the families we care for:

Accidentally hurting your baby is one of our biggest fears. No nurse goes to work thinking they want to hurt someone. None of us leave our house thinking, “I really want to make someone suffer.” There are a million and one ways a nurse can accidentally do something wrong. And every day, all day, we are very conscious of this fact and we work hard to provide the best care we possibly can… even if we’re short-staffed, even if our assignments are difficult, even if every room on our unit is full. Even though we literally have 20 things to do at any given moment with a handful of different, complicated patients, we strive to provide compassionate care in a timely manner while struggling to chart every single action we take. We know we’re going to make mistakes… our only hope is that the mistakes we make do not cause harm.

That nurse made a lot of right decisions. I’m just keeping it real — but seriously, that nurse could have made a lot of other really bad decisions. She could have dropped the baby and not told anyone. Even though she was probably frightened and distraught that her action caused a baby harm, she chose to do the right thing and immediately get the baby evaluated.

A nurse’s mistake can have many consequences. No one is asking why the nurse had the baby in the first place. I would bet any amount of money that she was trying to allow an exhausted mother to get a few minutes of uninterrupted sleep. And although I do not agree with this practice, I’m sure her intentions were pure. What people who are not nurses do not understand is that our mistakes can have many consequences. If we make a mistake, we can be peer-reviewed, which means our actions are brought before a committee to determine our nursing fate. We could lose our nursing license, leaving us unable to work or financially support ourselves or our family. If it’s deemed we were neglectful, criminal charges could be filed against us, and we could face hefty fines or even jail time. And our actions at work and at home are all up for examination and scrutiny.

That nurse is suffering right now. I don’t say this to diminish any anguish the family must feel that their baby was hurt while in the care of a healthcare provider. But wherever that nurse is right now, I promise you that she has been suffering. As I said before, no nurse goes to work wanting to hurt someone. She has had to endure being judged by her peers, questioning whether or not her facility would support her, and knowing that she caused a family distress. This is an incident that she will never forget, an incident that will probably taint her 30-year memory of nursing.

If you would have dropped your baby while in the hospital, the nurse would also be blamed. I don’t believe healthy mothers and healthy babies should be separated while in the hospital. I don’t believe a nurse should take a baby from a mother, even at her request, so that the mother can get uninterrupted sleep. This may not be a popular opinion, but as nurses, we need to see how these mothers interact with their babies even when they’re exhausted and sleep-deprived. But this leads to another issue… even if this mother would have dropped her own baby, the nurse and hospital would still be blamed. It would have been all about rounding and if it was documented that the nurse educated the patient not to sleep with the baby in the bed or if the room was free of clutter. As nurses, we have to be everything to everyone.

We are all human. As I drive to work tomorrow, I will think of the patients I will meet and care for. And as I walk through the doors of my hospital, I will think the same thing I have thought every single day since I graduated from nursing school: Just don’t hurt anyone. I know I will make mistakes. I’m human. But I hope I never make a mistake that hurts or kills someone. And that is a fear that lives inside of every nurse everywhere. My thoughts are with this family, and my thoughts are also with this nurse. To every nurse out there — May the mistakes we make tomorrow bring no harm to the patients we try to give so much to.

 

Advantages Of Being Bilingual in Nursing

Bilingual If you’re considering a career in nursing and are bilingual, this can be a tremendous advantage for you, your patients and their families. With increased diversity in the U.S., patients with limited English-language skills often arrive at the emergency room and there is no one available who speaks their language. This makes it very difficult for everyone involved to try to understand why the patient is there. Sometimes a family member who speaks limited English accompanies the patient and attempts to describe the family member’s symptoms. This is not an ideal situation and can lead to misunderstanding, frustration and an incorrect diagnosis. To drive this scenario home, imagine you’re on vacation in another country and become ill. You need medical attention, and when you arrive at the hospital no one understands you. This is a scary situation!

While most healthcare institutions offer translation services, sometimes the service is provided over the phone. This method is efficient in communicating information such as what the patient’s symptoms are, describing the appropriate course of treatment, or explaining the specific care of a condition at home. However, we all know there’s nothing like the ability to communicate with someone on a more personal, face-to-face basis. The patient may have more questions after the phone conversation is over. They or their family might ask questions such as, how often should I take this medication? Should I take it with or without food? Who do I call if I have questions when I get home?

As a nurse who is bilingual, you can be a tremendous help and source of comfort in answering these questions. Let’s take the example of a Hispanic nurse who not only speaks and understands both English and Spanish, but who also understands Hispanic culture, values and family traditions because of growing up in that community. My friend Esteban, who happens to be a bilingual Hispanic nurse, also knows the prevalence of certain diseases in the Hispanic community. These include diabetes, hypertension and cardiovascular issues. He’s seen these diseases in his family and community. He mentioned that diet and genetics contribute to these problems as the Hispanic diet often contains a lot of pork and fatty foods, which can lead to these conditions.

This is important information he already has because he is a member of the Hispanic community. He also speaks the language and can translate information to the medical team. His ability to communicate between the patient and medical team as well as his knowledge of Hispanic culture is extremely valuable in the care he can give his Hispanic patients. The ability of a patient to communicate directly and effectively with their healthcare provider increases feelings of trust and understanding, which can lead to a higher level of care and well-being. Again, I’ll take you back to becoming ill while traveling in another country and you don’t have the tools to effectively communicate your symptoms. Finding someone on the medical team who speaks English would be a tremendous relief!

The bottom line is clear: open communication, in terms of both verbal and listening skills, is essential to assessing a patient’s problem and determining the appropriate care and treatment. If you’re considering the field of nursing and are bilingual, you know so much already about your community’s language, customs, food and family values. You also have an awareness of healthcare issues prevalent in your community. As a bilingual nurse, you can be incredibly effective in delivering a high standard of care while putting your patient at ease.

The Advantages and Disadvantages of Nurses on Social Media

Social Media Social media can and is a wonderful and easy way to connect with people on a personal or professional basis. Most platforms are free and easy to use. It’s not difficult to find people and topics of interest. People can find and connect with their peers, family, and friends. They have unlimited access to the entire world.

Before anyone begins (or continues) their adventures into the world of social media, there are a few points to be aware of as a healthcare professional. With freedom comes advantages and disadvantages.

Advantages:

  • Connecting with anyone in the world. Simply Googling a word, topic, business, or an individual will give anyone complete access to anything they want to know. Whether they are blogging, using Twitter, Facebook, LinkedIn, or any other social media platform, there are people there to talk to.
  • Networking has never been easier. Sharing information and learning is at anyone’s fingertips. Whether they are talking to someone local or across the world, social media brings you together. Distance is no longer a problem.
  • Since 80% of all internet users are looking for health information, anyone can and will reach a large audience with their outreach and posts. People want information and Nurses have plenty to give them. They are able to correspond at their convenience and have the time to do so.
  • Education for Nurses and others is available. There are numerous groups and programs that offer classes and give them opportunities to learn and interact with their peers and other students.

Communication flows easily and in abundance on the internet and that’s also where the disadvantages come into play.

Disadvantages:

  • Privacy doesn’t truly exist on the internet. Regardless of any and all security features, once something has been posted, it’s there forever. Even deleting it doesn’t get rid of it. It is still stored somewhere in the clouds and is never truly gone. It’s crucial that a person think twice before posting. Screen shots cause the comment or post to remain forever on someone’s computer.
  • The risk of inaccurate information becoming a “fact” is common. It takes a bit of work and due diligence to research the accuracy of what you’re reading, but it’s worth the time. A person should only share or repeat what they themselves have thoroughly researched and confirmed as accurate.
  • Not HIPAA compliant. Very few software programs meet the strict guidelines for HIPAA regulations, including but not limited to Skype and Google Hangouts, texting and email. Unless the individual is discussing a patient as a Nurse and on a secure platform, one should never post anything about their patients.

Violating HIPAA regulations has resulted in healthcare professionals facing disciplinary actions such as:

  • Fines levied
  • Suspension from work
  • Being expelled from Nursing school
  • License revoked/fired
  • Criminal charges being brought
  • Incarceration

Abuses on social media as a Nurse are far reaching. Whether a Nurse violates HIPAA or behaves in an improper and unprofessional manner (arguing online, breaching patient confidentiality, harassment, etc.) can and does impact not only them, but their employer and their professional affiliations as well. This can also involve the state board and violate state laws.

Posting the pictures of patients, even when their identity is concealed, has resulted in the above actions being taken against a Nurse and other healthcare professionals.

The best rule of thumb with social media is this: Remember that what you are about to post will be accessible to 7 billion people, so post as if everyone will read it. Social media can be fun as long as you understand the good and the bad.

 

Nurturing the New Year

New Year The New year is here and the opportunities for a fresh start it brings shouldn’t be cast aside. Nurturing the opportunity as if it were a patient can yield surprising benefits both in and out of the job title. There is always room for improvement in a position that demands the hardest work for the sweetest reward, the gratitude of well-cared for patients.

Mind and Body

It is never too late to get personal health on the right track. Though the push for a healthier lifestyle should come from within, the simple fact is much like fitness instructors or your fellow doctors, patients ultimately expect those in health care roles to be healthy themselves. So whether it involves starting (and sticking to) that new gym membership or curbing poor eating habits, do your best to stand firm.

As a Nurse, the mind is just as important to nurture as the human body. With the continuous implementation of newer technology into the healthcare field, it can never hurt to be one step ahead by taking classes, particularly given the fact they look great on a resume. Focus on adding in new certifications such as those for operative nursing or ACLS, or expanding current skills. Regardless of qualms, continuing your education or taking the plunge on the journey to a BSN, MSN, NP or PhD is a worthy cause for the new year.

Tools of the Trade

Clearly technology is only as effective as the individual operating it, but that doesn’t mean Nursing staff don’t benefit from productive upgrades in the work space. How to Deal with the Stress of Nursing

Stress Nursing is one of the most stressful occupations in America. Nurses have higher rates of illness and psychiatric problems than other professionals. There are things Nurses and other health professionals can do to minimize the stresses of Nursing. The best way to do this is to look at individual stressors and find ways to minimize or cope with them.

Long Hours and Shift Work

Long hours are stressful both physically and mentally. According to USA Today, the long hours Nurses work have become such a problem that the American Nurses Association has made recommendations about how many hours Nurses should work. The recommendations include not allowing Nurses to work more than 12 hours a day. Shift work also causes a lot of stress for Nurses, and the AMA has also made the recommendation to minimize night shifts for Nurses working both day and night shifts.

Insufficient Resources

Having insufficient resources makes a nurses’ job more stressful, and it makes it harder for them to do their job properly. Insufficient resources take on different forms in different settings. Many times it is a shortage of Nursing personnel, which of course means the Nurses that are working have a higher workload. Other times, it is a lack of the material resources that Nurses need to do their job, whether it is due to budget cuts or oversight. Even small items like tape cause a big inconvenience when a Nurse has to search for it before she can draw blood or place an IV.

Resources can be improved by improving pay for Nurses and making sure there is enough room in the hospital budget for other necessities. Proper inventory keeping is also important. If a Nurse notices a shortage in a particular area, it is important to mention it. Hopefully items are ordered immediately and put in the appropriate paces.

Poor Reward System

Nurses are essential to the proper functioning of hospitals, and they work very hard. Yet many facilities don’t have any kind of reward system in place, nor do they take the time to tell Nurses they are valued and appreciated. Many times Nurses are taken for granted.

Studies have proven that rewarding employees for good behavior is essential to them being satisfied with their jobs and to retaining employees. Hospitals should have a rewards system in place. Nurses should also recognize each other for their hard work.

Bullying and Abuse

No one should have to endure bullying and abuse in the workplace. The ANA found that 17% of Nurses report being the victim of physical abuse at work, and 57% of Nurses reported being verbally abused or threatened. Physical abuse was usually perpetrated by patients or family members. Verbal abuse and bullying was usually at the hands of coworkers.

Hospitals should hold educational seminars about what constitutes verbal abuse and bullying. Human Resources employees should be available to mediate and help employees solve conflicts. To protect against violence from patients and their family members, it is a good idea to have some type of security presence in the hospital.

Lack of Communication

Lack of communication also causes stress for Nurses. Communication between Nurses and doctors is essential to a hospital running smoothly. Daily or weekly meetings are one way to ensure proper communication. A suggestion box can give employees an anonymous way to communicate suggestions.

Compassion Fatigue and Burnout

Compassion fatigue happens when a Nurse stops caring about their job and patients. Sustained stress over a long period of time can lead to compassion fatigue. Burnout occurs when a Nurse becomes depressed, withdraws from others, and feels fatigued.

To prevent compassion fatigue and burnout, try to take time off to relax. Don’t over extend yourself. Hospitals that have a reward system in place and make Nurses feel appreciated helps combat burnout. Everyone wants to feel appreciated in both our personal and professional lives.
